Why do women choose to get breast implants? It’s the number one cosmetic surgery in the U.S., even though it carries with it the burden of judgment and dismissal. So what’s driving these women? Are they young and insecure? Superficial? Desperate?

The answers might surprise you.

Of the 300,000 implant procedures in the U.S. in 2013 (up 37% since 2000 and still rising), the breakdown by age, according to statistics from the American Society of Plastic Surgeons, goes like this:

55+ — 6,934

Note the largest group: 30 to 39-year-old women. Not recent graduate from Uni. Not porn stars. Not young women on the prowl.

Most 30 to 39-year-olds are in the middle of their childbearing years. Many lose breast volume after pregnancy and aren’t happy with the way their boobs hang after major weight loss. There’s no way to get breast volume back, short of breast implants — no exercises or muscles that build them back up. So these women turn to implants to restore their original, and now lost, shape and size. They’re called “mummy makeovers,” breast implants combined with tummy tucks and liposuction.

Of the 85,000-plus procedures among the middle-aged population, 40-54, one can assume it has to do with staving off the march of time. There were also 92,000-plus eyelid surgeries in that age bracket. Of the nearly equal number of procedures among women in their 20s, statistics demonstrate that in that age group, 69,000 surgeries had to do with nose reshaping. So yes, there may be definite body dissatisfaction in this demographic, but it’s not just about breasts.

It’s hard to miss the media outrage over boob jobs handed out as high school graduation gifts. But, again, let’s look at the stats: board-certified plastic surgeonswon’t perform breast augmentations on women under 22, since breast tissue isn’t considered fully developed until that time (though breasts can and do change in size and shape all the way through lactation and past menopause).

Even the FDA doesn’t recommend breast implants for patients under 18. Yet sometimes such procedures are necessary for young women — to correct severe breast asymmetry, genetic chest deformities, or extreme under-development. And to put that 8,000 number above in perspective, nearly 6,000 young men in the same 13- to 19-year-old age group had breast-reduction surgery for gynecomastia (male breast enlargement) that same year. Another 30,000 teens (not broken down by sex) also had nose jobs. It’s likely that with young people these events are often more corrective and remedial than solely cosmetic.

Why do women want way bigger breasts?

What is extreme and what is “normal”? Women’s bodies come in all shapes and sizes, and their breasts don’t stay the same. Bra fitters and the intimate-apparel industry know the true size of women’s breasts, and bodies are rarely perfectly proportional. Some tiny women inherit outsized breasts, which make a much larger statement. Turns out that deep-cup bras with smaller bands (28 to 32 backs with DD to J cups) are the fastest-growing segment of the bra industry. These new sizes aren’t being marketed to young women with breast implants.

Breast implants aren’t measured by cup size but by cubic centimeter. Plastic surgeons know that a 300cc implant on one woman’s frame will look quite different on a taller or wider body. Look through the forums and you’ll find plenty of breast augmentation patients who go from a B to a C cup or a D — hardly super-sizing, although there are always extreme cases. And there’s only so much physical room to place an implant under skin or muscle. You can find plenty of examples of badly botched oversized boob jobs if you search “breast implant photos.” So no; most women who get breast implants are not shooting for the moon.

Isn’t it about getting male attention and approval?

Numerous studies attempting to show male preference for one size of breasts over another all seem to contradict one other. A 2013 article in Psychology Today cited a study that men who were poor and/or hungry tended to favor women with larger breasts (as did men interested in fatherhood), while men higher on the socio-economic scale, men who’d just had a meal, and men forestalling fatherhood tended to go for a smaller-breasted ideal. Another study claims sexist men prefer bigger boobs. Many larger busted women aren’t thrilled with the unwanted attention paid to their breast size. The bottom line? Men like breasts. They just do.

Shouldn’t women accept their breasts the way they are?

What is it about women’s breasts that make their surgical alteration more shocking than all the nose jobs, eye jobs, and facelifts combined? It’s hard to say. But women aren’t complaining, and report higher self-esteem an self-confidence after surgeries.

Some new enhancement procedures even use a woman’s own fat to build up her breast tissue (goodbye implants). Breast lifts (aka breast mastopexy) are outpacingbreast implants and have jumped by over 70%, with some 90,000 choosing the procedure in 2013.*** There were also 40,000 women who had breast reductions and 24,000 who had breast implant removal surgeries the same year. It’s safe to say women are choosing to control their breasts with surgery.

Isn’t it a matter of personal preference (combined with disposable income to fund plastic surgery)? If a woman wants to adjust the size or shape of her boobs, who are we — or anyone, for that matter — to judge otherwise?

How Big A Breast Implant Can I Have?

We all know by now that the size of breast implants is measured in cubic centimetres (ccs), and they increase the size of a woman’s breasts on average one cup size every 175 to 200 ccs. It all well an good saying that you would like to go from an A Cup to a DD, but physically can you? This is all determined by your Thailand Plastic Surgeon  based on a number of this one being, your anatomy and chest measurements that he will take during your consultation

Your key dimension for determining your breast size is the width of your breast. The breast width is measured (in cm) from the cleavage area to the outer part of your breast, next to your arm. This is measured in a straight line and is not curved around the arc of your breast. Most women’s breasts are about 11-14 cm wide.

Once your Surgeon has done your measurement and knows the width of your breasts you can now discuss with them the dimensions of breast implants that would be best for you. An implant slightly narrower or about the same width as the width of your breast is usually a good choice and should fit you well. So, if your breasts are 12 cm wide, you should choose a breast implant with a diameter of about 12 cm. If your implant choice is within 1/2 to one centimeter of your breast width, you should do well. But, it is best to select an implant that is slightly narrower, rather than wider than your breast. So, in the case of a 12 cm wide breast, an implant with a diameter from 11-12 cm should be satisfactory.

What limits how big a Breast Implant you should go?

  1. Your current breast size. This matters a lot because you’ve got to have enough existing breast tissue to cover the implant. If you haven’t got a lot to spare, some implants may just be too big to fit.
  2. Your chest and shoulder width. Fortunately, implants come in different base widths so you can get a size that works well with your frame.

Knowing and understanding the dimensional characteristics of breast implants by your surgeon is very important in determining which implant is best for you. In majority of Breast Implant surgery in Thailand, moderate profile implants are used and usually fit the average size patient best. Moderate profile implants tend to create nicely proportioned, natural appearing breasts.

Post Breast Augmentation Massage

Breast Implants are a foreign substance. Your body will naturally respond to an implant by creating scar tissue or a capsule. This capsule can become excessive; the results may include:

  •  Reduced size of the implant pocket
  • Restricted movement of the implant within the implant pocket
  • An unnaturally firm feeling to the breasts
  • A misshapen appearance to the breasts

Regular breast massage in the first 6 months following Breast Implant Surgery in Thailand can reduce the potential for these conditions by manipulating your implants so that they can move freely within the implant pocket.

Breast massage can also retain the contour and symmetry of your breasts, or to help gain breast symmetry where one implant may be slightly higher than the other.

When to perform breast massage:

The following exercises are the necessary steps to effective breast massage.

  • Begin the exercises 5-7 days after breast surgery, when it becomes tolerable to do so. Perform the entire set of exercises lying down at least 2 times each day for the first 3 months following surgery. While this may be uncomfortable when you first begin to perform the exercises, it is very important that you begin these exercises early. 
  • After 3 months, if your breasts feel soft and implants are moving freely, you may perform your exercises 1 times per day.
  • After 6 months and every day the first year, if your breasts feel soft and implants are moving freely, you may perform your exercises once per day or every other day.

Should you experience any sharp pain, hot or flushed breast skin, severe tightening or hardness at any time with your breast implants, please consult our office as soon as possible.

Note: In some cases, your Thailand Surgeon may recommend certain directions to not massage your implants. This will be discussed at your first post-op visit 7 days after your surgery in Thailand.

Note: Tear drop, anatomical, and/or textured implants are designed to adhere to the inner capsular tissues and, therefore, massage of these types of implants is not done, unless specified by your surgeon.

Do I need to Replace Breast Implants?

The rule of thumb or lets call it what it is the the Myth out there has been for a long time that breast implants should be changed every ten years, but is this really true?

Should breast implants be replaced at their tenth anniversary? If your breast implants are ten years old, is it time to change them? The question comes up in every Plastic Surgeon’s Office worldwide daily.

While it is unrealistic to expect that every breast implant will last indefinitely,  routine implant replacement every 10 years is neither necessary nor justified. Implants in some women may last many years while in others they need to be replaced on one or more occasions if there is a problem.  In essence, the exact life span of any implant is unpredictable

Why are Breast Implants Removed?
According to the FDA, breast implants are not considered lifetime devices. In order of occurrence, the most common reasons for breast implant removal are:
1) the patient’s desire to change implant size or style,

2) deflation and

3) capsular contracture. These three reasons account for about 90% of all the breast implants that are removed. Other less common reasons for implant removal and replacement include: implant malposition, wrinkling, palpability, breast pain and iatrogenic injury (inadvertently broken by a doctor). Not on the list: breast implants are ten years old.

If My Breast Implant are older are the More Likely it is to Fail?
If you are happy with your size, there is no indication of leakage, your breast remain soft and are not painful, the fact that your breast implants are ten years old is not a reason to have your implants removed or exchanged. The older the implant, the more likely it is to wear out, but most breast implants last longer than ten years.

For saline implants, leaks are pretty easy to determine. If saline filled breast implants leak, they flatten, and it is obvious that there is a leak. For the more common silicone gel filled breast implants, the shell of the implant can be damaged without any outward sign of leakage. This is termed a silent rupture. If you have silicone gel filled breast implants, the FDA recommends obtaining an MRI of your breasts at three years, and then every two years after surgery to check for a silent leak. If a leak is discovered, the implant should be removed, or replaced, to prevent gel migration and additional scar formation.

Breast Implant Warranties
The two most common Breast Implants used in any reputable Thailand Hospital are Mentor and SIlmed manufacturers both offering warranties on their breast implants. Both offer similar warranties, although the details vary by manufacturer and implant (saline vs. gel).

At the time of this post, both companies will replace a defective implant for free. Both companies will allow you to change style with the cost based on the differential between the current implant and the new style selected. If the new style costs more, you are charged only the difference in price in most cases. Financial support is also available during the first ten years to reduce the cost of the replacement surgery.

Breast Implant Revision Surgery
The bottom line is that most breast implants do not need to be replaced at ten years. If you have breast implant surgery in Thailand, it is reasonable to expect to need another surgery at some point in the future. The chances of having breast augmentation revision surgery within ten years of your initial breast augmentation is somewhere between 25% and 50%, and only about half of these breast augmentation revision surgeries involve replacing the breast implants. Reoperation rates are higher for breast reconstruction after a mastectomy. If your breast implant has deflated, be certain to check your warranty. Most deflated implants will be replaced at no cost, and you many be eligible for assistance to defray the cost of surgery if the replacement occurs within ten years of your primary breast augmentation.

You do need to be aware how your breasts can change over the years. As we age our skin and muscle and breast change, so yes the implants may last our lifetime, but in say 20 years they may not still look the way you want. Being aware of this you need to factor in that it is possible in the future that you may need another operation to either lift or replace your implants, not cause they are damaged, but due to natural causes.

In closing if there is no problem why should a woman be subjected to the inconvenience, risks and costs of further surgery when there is no good reason to do so?  If routine replacement every 10 years was important or beneficial, regulations would certainly exist to compel surgeons to replace every implant after 10 years – and clearly this is not the case!

What Do I Expect After Breast Implant Surgery

Getting breast implants  or any Breast Surgery in Thailand is a big decision, one that thousand of women make each year. Along with the joy of having larger breasts comes the responsibility of taking care of one’s body. There are many things that you should be considering as you recover from breast augmentation in Phuket.

You need to accept that, the recovery process is just as important if not more than having the surgery, since this is the stage during which the risk of complications can be minimised by the choice you make!

Before you even travel to Thailand to get your breast implant surgery, your Client Manager will help you plan out your recovery, which be done prior to your departure It is important that you do not over commitment yourself to anything beyond resting and recovering, especially for the first few days following the operation, since this is the time when you will need the most rest and assistance from your travel companion or Group surgery coordinator.

Immediately following your breast augmentation, your chest will be wrapped in a tight surgical dressing. While it may be uncomfortable, the snugness is intended to prevent excess swelling.

Your Thailand Plastic Surgeon will place one or more drainage tubes at the surgical sites, which will allow fluids to leave your chest during the initial stage of healing. Upon your discharge from the hospital you will receive instructions explaining how to take care of your incision sites over the coming week until your check up.

You remain in hospital overnight after surgery to ensure you are observed and assistance provided as required after surgery. Besides being in no state to return to your recovery hotel, you will be groggy, heavily medicated and in need of help when performing basic tasks.

Fortunately, you should be able to walk with minimal assistance. In fact, it is always recommended that women get up and take a brief walk every hour or two during the first day after the operation. This will reduce the risk of blood clots.

THE FIRST FEW DAYS

  • You may experience considerable discomfort. Breast augmentation incision sites can ache or throb, requiring use of prescription medications which will be provided to you at your discharge from hospital
  • Its normal to feel some stiffness, soreness, tenderness or itchiness of the breast during the first two to five days following surgery. Having the assistance of a friend can make it much easier to move around or manipulate objects.
  • You will probably feel fatigued and sore for several days after the breast augmentation surgery, so rest is essential.
  • It’s normal to experience a feeling of tightness in the breast area as your skin adjusts to your new breast size and your breast implants. You may also notice diminished feeling in your breasts and nipples during immediate recovery. Your plastic surgeon may recommend breast massage or other exercises to speed recovery and help alleviate any discomfort.
  • Your surgeon may ask you to wear a postoperative bra or compression wrap, for extra support and positioning while you heal.
  • It is common to experience bruising or discolouration after surgery. Typically, this goes away within days,within a month, post-operative swelling should be all but gone.

THE NEXT FEW WEEKS

  • When can you get up and starting moving again? Most surgeons will recommend doing regular walking and other light, non-strenuous activities within a day or two or surgery
  • Many women have trouble lifting their arms during the first week of recovery. Likewise, your surgeon will likely suggest that you avoid lifting, pulling or pushing anything, as well as exercising or doing anything that involves twisting your abdomen. Such activities can affect the speed at which your incisions heal.
  • It’s best to avoid any strenuous activities that could raise your pulse and blood pressure, or which require strenuous use of your arms and chest.
  • Your breasts may remain swollen and sensitive to physical contact for a month or longer.
  • Your surgeon will be able to provide you with more specific details about the recovery process. Contact your surgeon should any problems occur after your surgery.
  • Important note: If you experience a fever, or noticeable swelling and/or redness in your implanted breast(s), you should contact your surgeon immediately.
