If you chose a certain size breast implant, you would receive a predetermined amount of forward projection from that size implant. Round Breast Implants

Of the two breast implant shapes, the round breast implant is the most common type used for breast augmentation. A round breast implant is shaped like a flattened sphere. For women who desire fullness in the upper poles of their breasts, the round breast implant is a good choice.

Many women choose round implants because they tend to provide the greatest amount of lift, fullness, and cleavage. Tear Drop Breast Implants

There is great debate amongst surgeons which is better. Some studies have shown fewer complications – specifically capsular contraction – with textured implants, but others have shown the opposite. The shells of both saline and silicone gel breast implants are made of silicone elastomer (rubber), and the surface area is either smooth or textured. The main purpose of textured implants is to promote tissue adherence to the irregular surface and hold them into place. Therefore, a textured surface is used with contoured implants because they have a top and bottom, and rotation or movement distorts the breast shape. On the other hand, most round implants have a smooth surface because the breast shape is not affected if the implant rotates in the breast pocket.


Smooth Breast Implants

Smooth implants are used in roughly 90 percent of breast augmentation surgeries today. A smooth breast implant is able to move freely in the breast pocket in which it is placed. Many women  eel that smooth implants have a more natural feel and appearance than textured implants. Smooth implants usually have thinner shells than textured implants, which makes them the softer choice of the two. Textured Implant

It was originally thought that the textured surface of an implant could reduce the possibility of capsular contracture, a potential complication of breast implants. However, because evidence is not clear and there is no consensus as to whether a textured breast implant actually reduces the possibility of capsular contracture, many doctors prefer to use smooth, round implants. Smooth implants are often placed under muscle tissue in order to help avoid possible capsular contracture.

Textured breast implants have an irregular surface that is intended to hold the implant in place. The scar tissue that naturally forms around the implant conforms to the textured surface and “grabs” the implant to prevent it from being displaced or turning in the pocket, which could result in distortion of the breast shape. A textured surface is used with contoured implants to prevent movement because the teardrop shape is distorted if any rotation occurs. Textured implants were designed to lessen the risk of capsular contracture, which is an excessive contraction of the scar capsule that surrounds the implant, by preventing scar tissue from forming around the implant in a uniform manner. However, studies have proven inconclusive and no firm evidence exists to support the theory that textured implants reduce the risk of capsular contracture.

If a patient requires implant removal due to capsular contracture or another complication, it may be more difficult for the surgeon to remove the implant because of the adherence of tissue to the textured surface. The shells of textured breast implants are slightly thicker than smooth implants.Textured implants are more firm and carry an increased risk of rippling that is visible or can be felt through the skin, especially for women with smaller breasts. There is also a small increase in the chance that the implant shell will rupture.

This is easily measured by the surgeon with a calipers or measuring tape. The implant diameter must be less than the breast base, or it will be “squeezed” circumstantially. This will force it to scallop, or ripple around the outer edge, creating a range of other problems. Among these is visible rippling; It has been observed of much less rippling since the high profile implants became available, because the base diameter can be matched more precisely.

High profile implants have also become a favorite of many cosmetic surgeons and are recommended to many patients due to their excellent surgical results.

High profile implants project the further off the chest wall, when compared to other common implant profiles. The high profile implant will achieve this projection by having a smaller base diameter than an identically sized (in cc’s) moderate or low profile implant shell. By decreasing the diameter of the shell, but keeping the same fill volume, the manufacturer is able to increase the depth or forward projection.

High profile implants are perfect for women with a narrower chest frame who still want a considerable size increase during their breast augmentation surgery. Instead of using a wider base diameter moderate implant, the doctor will choose a narrower high profile shell which will suit the patient’s anatomy and project out to accommodate the volume of the filler material. High profile implants are also commonly indicated for patients who are undergoing a combination breast enlargement and donut mastopexy procedure. This modified mastopexy, like many breast lift operations sometimes squares off the breast shape slightly, making it appear wider and less round. The shape of the high profile implant shell restores the rounded shape of the breast, while still allowing the considerable benefits of the mastopexy lift.

While some studies say that these high profile shells reduce visible wrinkling and rippling, the reasoning given is generally incorrect. It is not anything to do with the design of the implant which decreases this noticeable unwanted breast enlargement complication. it is simply a better anatomical fit. Wrinkling and rippling often occurs when a women with a narrow chest frame desires large implants. The base diameter of her natural breasts is smaller than the base diameter of her implants. Obviously, the implant will be compressed and distorted by the small confines of the pocket inside her breast. This distortion causes the wrinkling and rippling which can be so unappealing to many affected patients. By utilizing a proper fitting high profile implant with a base diameter smaller than the natural breast width, the surgeon can guarantee a good fit with no implant shell distortion. The patient’s size request is accommodated by the increase in forward projection creating a flawless augmented, butvery natural looking breast.

Ultra High Profile

Instead of using a wider base diameter implant, the doctor may choose the ultra high profile if the high profile is not enough projection. This may suit the patient’s anatomy and project out to be able accommodate the volume of the filler material.

There are two ways to increase projection in an implant. The first way is to add more volume to the implant. The other way to add projection is to make the base (diameter) of the implant smaller. Can breast implants make it harder to find breast cancer during mammography?

Yes. Breast implants can obscure mammogram images, decreasing the ability of mammograms to reveal breast cancer. Still, studies show that mammograms are an effective way to screen for breast cancer in women with breast implants.

Women with breast implants should continue to have mammograms. (A woman who had an implant following a mastectomy should ask her doctor whether a mammogram of the reconstructed breast is necessary.) It is important to let the mammography facility know about breast implants when scheduling a mammogram. The technician and radiologist must be experienced in performing mammography on women who have breast implants. Implants can hide some breast tissue, making it more difficult for the radiologist to detect an abnormality on the mammogram. If the technician performing the procedure is aware that a woman has breast implants, steps can be taken to make sure that as much breast tissue as possible can be seen on the mammogram. A special technique called implant displacement views may be used.

If you have breast implants, you can take steps to make your mammogram more successful, such as:

Find a facility that sees many women with breast implants. Ask your doctor for a referral to a clinic where radiologists have experience performing and reading mammograms of women with breast implants. Ask about the clinic staff’s experience when you make your appointment. You’ll want to know that the clinic obtains special mammogram views in addition to the standard mammogram images to better evaluate the breast tissue.
Speak up about your breast implants at your appointment.When you make your appointment and again when you arrive at your appointment, tell the staff that you have breast implants.

There’s a slight risk that your breast implants may rupture during a mammogram. But the benefits of mammography in the early detection of breast cancer outweigh this risk.

Will the pressure of a mammogram cause an implant to rupture?
This occurrence is obviously quite rare. So, implants should not be a disincentive to getting a mammogram. Some patients ask if ultrasound, which is a gentler procedure, can be used to screen for cancer in lieu of mammogram. The surgical answer is no. Ultrasound doesn’t pick up the minute calcifications of early breast cancer. And it is very operator dependent; results vary tremendously based on who is performing and viewing the image. Ultrasound is an adjunct test used to distinguish between solid and cystic masses or to help assess very dense breasts. The current practice is not to use it as a primary screening tool to detect breast cancer in women without or with breast implants.

Does having Breast Implant increase higher incidences of breast cancer?
No. Many studies have found that breast cancer incidence in women who have had implants is actually lower than that of the general female public. One study from Sweden followed 3,400 women who had received cosmetic breast implants between the years 1965 and 1993. They had an average follow-up of 18 years. The women with implants were found to have a 30 percent reduced risk for breast cancer when compared to women without implants. Another study, this time in the U.S., compared breast cancer incidence rates among 13,000 women who had breast implants with expected rates based on U.S. surveillance. And here too the risk was slightly lower. It’s possible, though many feel not medically probable (or even statistically proven) that breast implants may decrease breast cancer incidence by:

Compressing breast tissue and interfering with its blood supply.
Decreasing temperature in the breast.
Stimulating an immunological response, which helps destroy potential breast cancer cells.

It may be, however, that the apparent decreased incidence of breast cancer in women with breast implants reflects the fact that these women are more likely to be thin (obesity is a significant risk factor for breast cancer). Also, the women with implants tend to be in a higher socioeconomic group with better nutrition, exercise, and cancer screening. They see their physicians, especially the breast specialist more frequently, have more frequent mammograms and better access to medical care than their cohorts who don’t have implants.

When cancer is found in women with implants, is the disease at a later stage?
This has been addressed in another study published in The Journal of the American Medical Association (JAMA), which used the data collected from the National Cancer Institute funded Breast Cancer Surveillance Consortium (BCSC). They compared 141 women with augmentation and nearly 21,000 without who were diagnosed with breast cancer between 1995 and 2002. The tumors were diagnosed at a similar stage and size, but tended to be lower grade (less worrisome) for women with breast augmentation compared to those without. The authors concluded that “although sensitivity of screening mammography is lower with breast augmentation, there’s no evidence that this results in more advanced disease at diagnosis compared with women without augmentation.”

How Big A Breast Implant Can I Have?

We all know by now that the size of breast implants is measured in cubic centimetres (ccs), and they increase the size of a woman’s breasts on average one cup size every 175 to 200 ccs. Your key dimension for determining your breast size is the width of your breast. The breast width is measured (in cm) from the cleavage area to the outer part of your breast, next to your arm. This is measured in a straight line and is not curved around the arc of your breast. Most women’s breasts are about 11-14 cm wide.

Once your Surgeon has done your measurement and knows the width of your breasts you can now discuss with them the dimensions of breast implants that would be best for you. An implant slightly narrower or about the same width as the width of your breast is usually a good choice and should fit you well. So, if your breasts are 12 cm wide, you should choose a breast implant with a diameter of about 12 cm. If your implant choice is within 1/2 to one centimeter of your breast width, you should do well. But, it is best to select an implant that is slightly narrower, rather than wider than your breast. So, in the case of a 12 cm wide breast, an implant with a diameter from 11-12 cm should be satisfactory.

Breast Implants are a foreign substance. Your body will naturally respond to an implant by creating scar tissue or a capsule. This capsule can become excessive; the results may include:

  •  Reduced size of the implant pocket
  • Restricted movement of the implant within the implant pocket
  • An unnaturally firm feeling to the breasts
  • A misshapen appearance to the breasts

Regular breast massage in the first 6 months following Breast Implant Surgery in Thailand can reduce the potential for these conditions by manipulating your implants so that they can move freely within the implant pocket.

Breast massage can also retain the contour and symmetry of your breasts, or to help gain breast symmetry where one implant may be slightly higher than the other.

When to perform breast massage:

The following exercises are the necessary steps to effective breast massage.

  • Begin the exercises 5-7 days after breast surgery, when it becomes tolerable to do so. Perform the entire set of exercises lying down at least 2 times each day for the first 3 months following surgery. While this may be uncomfortable when you first begin to perform the exercises, it is very important that you begin these exercises early. 
  • After 3 months, if your breasts feel soft and implants are moving freely, you may perform your exercises 1 times per day.
  • After 6 months and every day the first year, if your breasts feel soft and implants are moving freely, you may perform your exercises once per day or every other day.

Should you experience any sharp pain, hot or flushed breast skin, severe tightening or hardness at any time with your breast implants, please consult our office as soon as possible.

Note: In some cases, your Thailand Surgeon may recommend certain directions to not massage your implants. This will be discussed at your first post-op visit 7 days after your surgery in Thailand.

Note: Tear drop, anatomical, and/or textured implants are designed to adhere to the inner capsular tissues and, therefore, massage of these types of implants is not done, unless specified by your surgeon.

What Do I Expect After Breast Implant Surgery

Getting breast implants  or any Breast Surgery in Thailand is a big decision, one that thousand of women make each year. Along with the joy of having larger breasts comes the responsibility of taking care of one’s body. There are many things that you should be considering as you recover from breast augmentation in Phuket.

You need to accept that, the recovery process is just as important if not more than having the surgery, since this is the stage during which the risk of complications can be minimised by the choice you make!

Before you even travel to Thailand to get your breast implant surgery, your Client Manager will help you plan out your recovery, which be done prior to your departure It is important that you do not over commitment yourself to anything beyond resting and recovering, especially for the first few days following the operation, since this is the time when you will need the most rest and assistance from your travel companion or Group surgery coordinator.

Immediately following your breast augmentation, your chest will be wrapped in a tight surgical dressing. While it may be uncomfortable, the snugness is intended to prevent excess swelling.

Your Thailand Plastic Surgeon will place one or more drainage tubes at the surgical sites, which will allow fluids to leave your chest during the initial stage of healing. Upon your discharge from the hospital you will receive instructions explaining how to take care of your incision sites over the coming week until your check up.

You remain in hospital overnight after surgery to ensure you are observed and assistance provided as required after surgery. Besides being in no state to return to your recovery hotel, you will be groggy, heavily medicated and in need of help when performing basic tasks.

Fortunately, you should be able to walk with minimal assistance. In fact, it is always recommended that women get up and take a brief walk every hour or two during the first day after the operation. This will reduce the risk of blood clots.

After your  Breast Implant surgery in Thailand is complete, you will begin the recovery process. It’s important that you take good care of yourself during this time to ensure that your body has time to heal and look its best. Once you have made it through the first 24 hours of your recovery following breast augmentation, you are well on your way to healing and getting back on your feet. Read on to learn what to expect in the days and weeks following your surgery.

  • You may experience considerable discomfort. Breast augmentation incision sites can ache or throb, requiring use of prescription medications which will be provided to you at your discharge from hospital
  • Its normal to feel some stiffness, soreness, tenderness or itchiness of the breast during the first two to five days following surgery. Having the assistance of a friend can make it much easier to move around or manipulate objects.
  • You will probably feel fatigued and sore for several days after the breast augmentation surgery, so rest is essential.
  • It’s normal to experience a feeling of tightness in the breast area as your skin adjusts to your new breast size and your breast implants. You may also notice diminished feeling in your breasts and nipples during immediate recovery. Your plastic surgeon may recommend breast massage or other exercises to speed recovery and help alleviate any discomfort.
  • Your surgeon may ask you to wear a postoperative bra or compression wrap, for extra support and positioning while you heal.
  • It is common to experience bruising or discolouration after surgery. Typically, this goes away within days,within a month, post-operative swelling should be all but gone.


  • When can you get up and starting moving again? Most surgeons will recommend doing regular walking and other light, non-strenuous activities within a day or two or surgery
  • Many women have trouble lifting their arms during the first week of recovery. Likewise, your surgeon will likely suggest that you avoid lifting, pulling or pushing anything, as well as exercising or doing anything that involves twisting your abdomen. Such activities can affect the speed at which your incisions heal.
  • It’s best to avoid any strenuous activities that could raise your pulse and blood pressure, or which require strenuous use of your arms and chest.
  • Your breasts may remain swollen and sensitive to physical contact for a month or longer.
  • Your surgeon will be able to provide you with more specific details about the recovery process. Contact your surgeon should any problems occur after your surgery.
  • Important note: If you experience a fever, or noticeable swelling and/or redness in your implanted breast(s), you should contact your surgeon immediately.
